Once you have completed and submitted this form:

1. Print out two copies
   • A copy to put into the instrument case
   • A copy for your records
2. Please remove any extra items from the case (i.e. reeds, oils, rosin, etc.)
3. Give the instrument (with the copy of the repair request) directly to your student’s music director.
   Leaving it with your director, and not the front office, will help ensure the safety of your instrument, plus expedite your repair
